Welcome to the Community Monitoring of Health Services under NRHM Database site. This database site has been created to monitor the Community Based Enquiry of Health services under National Rural Health Mission. The process of Community Monitoring is taking place across nine states (Assam, Jharkhand, Chhattisgarh, Madhya Pradesh, Rajasthan, Maharashtra, Tamil Nadu, Karnataka and Orissa).
The site has been divided into two parts:

Activity Section (state activities, district activities, block activities, PHC activities and community/village activities)

Report Card Section (village report card, sub-centre report card, PHC report card, block report card and district report card). Cumulative report card can be viewed for each sub-centre, PHC, block and district.
The report card has three color codes on the basis of following parameters:

Green = 75% to 100%

Yellow = 50% to 74%

Red = 1% to 49%
